Titan submarine report: ignored warnings, almost no control

Carbon Fiber Submarine Lost During Titanic Expedition

In June 2023, a carbon fiber submarine, destined for the wreckage of the Titanic, sank in the Atlantic Ocean. The submarine, carrying five passengers, lost communication two hours into its journey. The submersible, operated by OceanGate, was undertaking an expedition to the site of the Titanic, which sank in 1912 following a collision with an iceberg.

Over 1,500 people perished in the disaster, which involved the ocean liner and a crew of approximately 2,200. A subsequent report released by the Canadian Transport Safety Board (TSB) identified multiple design flaws within the submarine and broader issues with OceanGate’s operational culture as contributing factors to the incident. The report, published on Wednesday, detailed numerous shortcomings in the submarine’s construction.

Prior to the fatal sinking, the submarine was reportedly generating a significant noise level. The TSB’s investigation highlights critical concerns regarding the design and oversight surrounding the operation of the submarine and its journey to the ocean depths.
